0.21 release cycle

[David Bremner]

I'd like to get a release out as it's been more than six months. Here's
my proposed schedule.

2015/10/15: initial release candidate (tagged as 0.21_rc0)
2015/10/22: hard freeze (only serious bug fixes)
2015/10/31: release
