[PATCH 0/5 v4] reworked crypto toggle, plus a few other toggles

- Don't retain the state for '='.
- If refreshing the view removes the previously current message from
  view, leave the cursor at (point-min).
- Adjust the display after refreshing if the previously current
  message is found.
